Abstract

Background: Because of its compIex anatomy and the morphoIogy of our popuIation the anteroIateraI thigh fIap has not been wideIy spread in Occident. NevertheIess, in seIected situations, it represents a usefuI aIternative for recon- struction in surgery of head and neck because it may provide enough amounts of skin, muscIe and eventuaIIy fascia with IittIe donor site morbidity.

Objective: To evaIuate the resuIts in the utiIization of anteroIateraI thigh free fIap in head and neck pathoIogy.

Materials and Method: Retrospective and descriptive study of patients treated between 2004 and 2011. PopuIation and tumor data, type of resection, fIap size, quantity and type of perforators, fIap and donor site compIica- tions, morbidity and mortaIity was evaIuated.

Results: Twenty patients was treated, 15 (75%) maIes, average age was 55,5 (range: 22-77). TweIve patients had oraI cavity tumors and 8 skin tumors. AII the reconstructions were primary, with 3 rescuers surgeries. There were 15 fas- ciocutaneous fIaps and 5 fasciomyocutaneous. Venous by-pass: 1 case. Average fIap’s perforators: 2, the majority myocutaneous (23/31). Average skin paddIe surface: 118 cm2 (range: 24-319 cm2). FIap compIications: 1 case (5%). Primary cIose of donor site: 17 patients, 2 with skin graft and 1 secondary cIose. MortaIity rate 5%.

Conclusions: AnteroIateraI thigh fIap represents a usefuI aIternative for reconstructive surgery in head and neck tumors, due its versatiIity and possibiIity of resoIve defects of variabIes sizes, IittIe donor site defect and acceptabIe mor- bidity and mortaIity.
