Abstract

Background: Diastasis recti associated with midline hernias is a recognized risk factor for hernia recurrence and impaired core function. In overweight patients, repairing only the hernia defect may be insufficient.
Objective: The aim of this study was to evaluate the preliminary clinical and functional outcomes of the eMILOS (Mini or Less Open Sublay) technique in patients with ventral hernias associated with diastasis recti.
Material and methods: We performed a retrospective observational study of patients undergoing the eMILOS technique between July 2024 and November 2025. The clinical and surgical data were obtained from the electronic medical records. Clinical follow-up was conducted through physical examination and targeted questions to assess hernia recurrence, diastasis recurrence, and functional and aesthetic satisfaction.
Results: A total of 16 patients underwent surgery; mean age was 56 years, and 14 were male. During an average postoperative follow-up of 8 months, no hernia recurrences were observed, but three patients experienced a diastasis recurrence. Two minor complications occurred (Clavien-Dindo grade 1). All patients reported functional satisfaction.
Conclusions: The eMILOS technique proved to be a feasible minimally invasive approach for comprehensive midline repair in patients with abdominal hernias associated with diastasis recti. The preliminary results are encouraging, and larger series with long follow-up are required.
